If the IRB asked you to resubmit your application with changes, your faculty advisor, the DRC (Dr. Waller), and Department Head (Dr. Fairbrother) do not have to review and sign-off a second time after the initial sign-off. The key is to fully address all of the provisos before resending it directly back into the system.
If, however, you did not include the faculty advisor in the routing and sign-off in the initial submission, you will need to reroute the application through the system and get your adivsor’s sign-off.
