FINAL REMINDER
Please submit your Travel Fund applications for travel January 1, 2018 to April 30, 2018 to Danielle Johnson (dmarti59@utk.edu) by Monday, November 20th, 2017. Important! This is the final due date for the College. I will need to get Dr. Bassett’s signature before I can send. Please plan accordingly.
When submitting these materials, please be sure to have the following:
-If applying for the Graduate Student Senate (GSS) Travel Award: (Application and information can be found at http://gss.utk.edu/travel-awards/ )
- Application and signature page (with applicant and faculty advisors signature provided)
- Supporting documentation (acceptance letter or abstract)
- Other Travel Request forms (below)
–Complete Other Travel Request forms:
- Travel request form (T-18) (located online at http://cehhs.utk.edu/for-staff/ or in KRSS office, HPER 322)
- CEHHS graduate student travel form (located online at https://cehhs.utk.edu/for-staff/)
- FOR GTAs- Class Coverage Form (attached)
**I will obtain the Department Head’s signature and submit to the College on your behalf.
Applications turned in after the deadline will not be considered. Please let me know if you have any questions.
